Name Change Requests
The VCUCard Office will issue you a VCUCard with your legal name entered into the database by either the Office of Records & Registration or the Human Resources department. To request a different name displayed on your VCUCard, please review the information below. Patrons are strongly encouraged to submit these types of requests prior to receiving their first VCUCard.

Your department Personnel Administrator (PA) will need a copy of your new Social Security card and an updated Personal Data Form (Word) indicating the legal name change. Your PA will review your documents and provide the appropriate paperwork to Human Resources.
Records and Registration will need a copy of your new Social Security card (or applicable documentation) and an updated Personal Update Form. The form should be returned to the Office of Records and Registration in Harris Hall, room 1100. If you are both an employee AND a current student, please also provide your new Social Security Card and updated form to your department Personnel Administrator (PA) and your PA will send the appropriate documentation to Human Resources.
Once your information has been updated, you can visit one of our offices to exchange your current VCUCard with an updated one at no cost. Please allow a minimum of three (3) business days for the change to take effect. You can also call the VCUCard Office during normal business hours to confirm this update.
If you are in gender transition or no longer identify with your legal gender, you may submit a Name of Use Change Request form to notify the VCUCard Office that you would like to change the legal first name that will appear on your VCUCard to the gender affirmed first name you identify with. You are strongly encouraged to submit this request prior to receiving your first VCUCard. Your request will be reviewed and further instructions will be provided within 3 business days. Requests for name changes that are profane, obscene, or derived from hate/speech will be denied.
If you currently have a VCUCard and your request is approved, you will be required to surrender your current VCUCard in exchange for the new one.
